Application Forms Available For On-line Submission

Internet Explorer Required

Most forms used by the District may be filled out and submitted on-line using the links below. However; applications will not be fully processed until the appropriate application fee has been received. Submitting the forms on-line can increase the speed and accuracy of the process by eliminating the need to interpret handwritten entries and smudged faxes. Each form you submit on-line will generate an "Application Received" acknowledgement that will be sent to the email address you provide on the form.
 

Registering a Well

New Registration - All wells have to be registered prior to drilling.  NOTE: A well serving only a Single Family Home does not have to be permitted, but it does have to be registered. If the well is for anything other than a single family home, you will also need to submit a New Well application from the section below.

Initial Permit Application

New Well - NOTE: (Includes form for emergency applications) Use this form only if the well has not yet been drilled.

Emergency Application Only – New Well Application Previously Filed

Original Application of an Existing Well - The well has already been drilled, but has never been permitted.

Previously Permitted - The well was permitted sometime in the past but does not have a current permit term.

Amending a Current Permit

Allocation Increase - You require more water than the amount for which you initially applied. NOTE: You must submit this application and have a new permit issued for the increased amount BEFORE you over pump the amount on your permit!

Allocation Decrease - You do not require the amount of water for which you initially applied. NOTE: This can only be used prior to payment of the Permit Fee. If the Permit Fee has already been paid, use the Rebate form at the end of the permit term.

Transfer of Ownership - Use this form if you no longer own the well in question.

Renewing a Current Permit

Renewal - Used to renew your permit term for the next permit term. We will automatically send you a renewal form approximately 4 months prior to the end of your current permit term. NOTE: If you wish to submit the form on-line, you must fill it in using the information from the form we send you.

Rebates

Application for Permit Fee Rebate- Use this to receive a rebate of your Permit Fee for an unused portion of your allocation after your permit term has ended. NOTE: Must be submitted within 90 days after the term expires.

Application for Groundwater Credit Rebate- To be used after your permit term has expired, to receive the unused portion of your groundwater credits.  NOTE: Must be submitted within 90 days after the term expires.

Application for Over-Conversion Credit Rebate- To be used after your permit term has expired, for permittees in Regulatory Area 3 who exceed the District's groundwater reduction requirements.

Reports You May Need To File

Annual Report of Water Pumpage - Used for reporting your pumpage for a given calendar year

Affidavit of Meter Calibration Test - Required as follows:

Meter Diameter   Testing Interval
Less than 6 inches   Every 3 years
6 inches and greater     Every year
PD “Positive Displacement” Type meters all sizes    as required

Meter Card Report – Used for reporting meter readings as required by the District (Rule 8.7).

Online forms are only 100% Compatible with Internet Explorer.  Other web browsers are not supported.   If you have problems with Internet Explorer load your browser without add-ins and try again.  There is absolutely an issue with Google Chrome browser and it will not work with these forms, other browsers may work but are not guaranteed.  As an alternative you may fax your form to 281-218-3700 or email it in .pdf format to info@subsidence.org with the subject Annual Report.
